When I was young, Wim Hof didn’t yet have the method, just the madness. In 2000, the year he swam 188.6 feet under the ice – his corneas freezing and a rescue diver pulling him out – I was waiting for a growth spurt I’d been promised by a loose-lipped pediatrician. I was also killing time. I did so unproductively by finding various ways to frighten myself. Deep, cold water made the list. Like many kids in Maine, I had access to a beater of a boat, which meant my friends and I also had access to open water.

We dared each other to jump in. Everyone did.

I think about what happened next every morning I walk out onto my roof in Brooklyn and slide slowly into my Cold Plunge, which marinates stubbornly at 45 degrees, even in summer as the roof sealant starts to ooze. It’s a sharp thrill, a neurological jolt followed by a deep full-bodied cramp, followed by adrenaline and a shot of awareness that tastes as clean as a cocktail – cerebrospinal fluid served on the rocks. I remember being a kid and almost shooting out of the water, scared of sharks that wouldn’t manage to make it that far north for another two decades. But you can master yourself.

Daring and discipline aren’t the same, but they crew the same ship.

The big question when it comes to the Cold Plunge, a well-made mini-pool that comes with a chiller and a set of instructions spiritually adjacent to a hot plate user manual, is whether it’s just a feeling. Does hopping in cold water and stressing the body (while engaging in controlled breathing) build immunity or lower inflammation? That’s the theory. That’s the so-called Wim Hof Method. There’s some scientific evidence to support it, but it has far more anecdotal support.

There are legions of cold plungers at this point. This chattering class is big enough to capsize a glacier or alienate a pod of porpoise with their enthusiastic evangelism A cursory search on TikTok, where the practice has become a bona fide trend, shows 1.4 billion views associated with the term “cold plunge.”

I’m not a true believer in Wim Hof or, to be fully honest, a true skeptic. I sit between ambivalent and agnostic. What I really am is nostalgic and also keenly aware that my old rise-and-grind routine with the coffee maker wasn’t working. If coffee doesn’t make me an asshole, it certainly reveals the asshole within.

The Cold Plunge is, considered closely, a reverse coffee. It’s a cold cup into which I pour myself — with results that are far more profound. Every morning I dare myself – memories of the transom of a Boston Whaler buoy to the surface – and then I do the thing. I emerge panting from the pain and the shock of it, but feeling in control of myself. Am I stronger or healthier for it? Hard to say in the absence of a control group, but I believe that I derive a psychological benefit from frightening myself. And, yes, the cold is frightening even when contained. The body isn’t built to trust.

The product itself, the slick Cold Plunge tub, feels like something of a totem. A Cold Plunge is a sure sign that you’re about to have a conversation about the benefits of psychedelics or Burning Man. I’ve never been to Burning Man (people who grew up on beaches distrust the desert.) But I don’t mind being mistaken for a bit of a searcher. I don’t believe in alternative medicine, but I do believe in trying stuff. And the Cold Plunge has been a positive trend for me, personally.

Will it work for everyone? No. Is it expensive? Absolutely, though not profoundly given the equipment involved. Am I convinced that it will make me live a longer, fuller life? Nah, but I still recommend it to friends for whom the cost won’t be prohibitive. It makes the morning more interesting and one dare naturally leads to the next.

Then, there’s what kind of mask to buy. When it comes to respirators, remember that N95s and KN95s are functionally the same device. “N95” is the certification in the US, while “KN95” is the certification in China, but both masks are respirators that filter roughly 95% of airborne particles. In fact, the CDC has used the two terms interchangeably. Meanwhile, the KF94 is the Korean certification and filters 94% of particles.

What the Experts Say

The experts SPY consulted for this piece all said that it’s less about where one purchases their mask and more about the type of mask they buy. Most major vendors have both trustworthy and counterfeit masks on their platform, so it’s up to the consumer to choose wisely. 

When it comes to which masks are “best,” studies have shown that respirators provide the greatest protection against COVID-19 infection. Anything is better than nothing, according to Emily Volk, president of the College of American Pathologists. “Even just a barrier mask can reduce the risk of spreading upper respiratory illnesses,” she said. 

That can include a surgical mask (most commonly seen as the disposable blue mask). These do not necessarily provide as much protection as respirators, especially when it comes to small particles in the air. However, they are a good option, said Dr. Amesh Adalja, a senior scholar at the Johns Hopkins Center for Health Security. In most cases, he said, he feels comfortable wearing a surgical mask even when he’s seeing patients infected with COVID. 

Regardless of what a person chooses, it’s critical that the mask fits them properly, said Dr. Adalja. That means it fully covers the nose, mouth, and chin without leaving large gaps between the edge of the mask and the face. “If it’s fitting snugly around the face, it’s probably performing the way you want it to,” he said.

The N95 mask was center stage the entire pandemic, and for good reason, but there are other protective masks out there that are easier to find. N95 masks offer superior protection over cloth masks and are regulated by the Center for Disease Control (CDC) as a medical-grade barrier against airborne viruses. KN95 and KF94 masks are also available for purchase online, and are sometimes easier to find than N95s, which begs the question: do they offer the same level of protection? And what’s the difference anyway?  

There’s essentially no difference between KN95 and KN94 respirators and very little difference between them and N95 respirators. While N95 is the terminology used in the U.S. — specifically by the National Institute for Occupational Safety and Health (NIOSH), which is part of the CDC — “KN95” is the terminology used in China. KN95s tend to stick out further from your face than an N95, but both respirators filter out around 95% of airborne particles. In fact, the CDC has used the two terms interchangeably. Meanwhile, “KF94” is the terminology used in Korea and refers to a respirator that filters out 94% of airborne particles.  

That’s a difference of one percent, which may not be especially meaningful, said Emily Volk, president of the College of American Pathologists. “Any mask that can effectively filter out viral particles can be a very helpful tool in combating the spread of this illness,” she said. Research confirms that wearing a mask is better than not wearing a mask, and the most important difference isn’t between types of respirators. It’s between respirators, surgical masks, and cloth masks. 

In 2021, the CDC studied the odds of testing positive for COVID while wearing no mask, a cloth mask, a surgical mask, and an N95 / KN95 respirator. The respirators were the most effective, decreasing the odds by 83%, followed by surgical masks (66%), and cloth masks (56%). However, there’s a caveat with this data, said Dr. Rebecca Choudhury, an assistant professor of medicine (infectious diseases) at the Icahn School of Medicine at Mount Sinai.

“Wearing an N95 or KN95 is not automatically better for every individual or situation,” she said. “For some, particularly those with facial hair, they may have trouble finding a respirator mask that can form an airtight seal to their face.”

Masks continue to be one of the best ways to mitigate risk. But to be effective, they don’t necessarily need to be N95 or KN95, said Adalja. “In most cases, I’m comfortable wearing a surgical mask when I’m seeing a patient with COVID,” he said. Homemade masks are also a good choice, as long as they’re maintained properly, said Dr. Rebecca Choudhury, assistant professor of medicine (Infectious Diseases) at the Icahn School of Medicine at Mount Sinai.

Dr. Choudhury discourages people from wearing a surgical mask underneath a KN95 or N95 mask because it breaks the airtight or nearly airtight seal that the latter forms. While medical providers may wear surgical masks over N95s, that’s to extend the use of the N95. “It makes no difference in the effectiveness of either mask,” she said. 

Consumers should also be vigilant about what they’re buying. The Centers for Disease Control and Prevention (CDC) estimates that up to 60% of KN95 masks sold in the U.S. are counterfeit or misrepresent the level of protection they offer. Genuine N95 respirators are approved by the National Institute for Occupational Safety and Health, which maintains a database of all TC approval numbers and lists several known counterfeit masks and manufacturers on their website.

The world is still feeling the impact of COVID-19 variant Omicron, although the initial wave has passed. Why? Current COVID strains circulating are all descendants of Omicron, and the most common one in the U.S. is XBB. 1.5 (also known as the “Kraken” subvariant). For that reason, Dr. Choudhury recommends that people who want additional protection, find themselves in higher-risk situations like traveling or attending crowded events, or have medical conditions that increase their risk of severe infection wear a mask. 

There are many styles and brands of masks that are effective, says Dr. Choudhury, but wearers should keep four things in mind: First, the most important quality in mask selection is fit. A proper seal is so important that health providers often undergo fit testing for their masks, said Dr. Amesh Adalja, a Senior Scholar at the Johns Hopkins Center for Health Security. The process involves putting masks on of various sizes and spraying a scent into the air. “If the N95 is sitting appropriately, you won’t be able to smell it.” 

The second consideration is that cloth masks work, but only if used correctly. Dr. Choudhury notes that cloth masks should be washed after every use, so having more than one to switch between is essential. Cloth garments like bandanas and neck gaiters are also not adequate substitutes for real masks since they don’t prevent wearers from inhaling or spreading viral particles. 

While a person can wear too few layers of protection, they can also wear too many, says Dr. Choudhury. She discourages people from wearing a surgical mask underneath a KN95 or N95 mask because it breaks the airtight or nearly airtight seal those masks form. While medical providers may wear surgical masks over N95s, that’s to extend the use of the N95. “It makes no difference in the effectiveness of either mask,” she says. 

Finally, she says to beware of counterfeit respirators. America has a mental health problem. According to the National Institute for Mental Health (NAMI), more than one in five adults in the US experience mental illness every year, and rates of anxiety and depression increased 25% during the pandemic. For men, the situation is especially dire, made worse by the fact that they’re less likely to seek out help or opt for prescription treatment. 

While women are twice as likely to experience depression, men are less likely to be medicated for it. Between 2015 and 2018, antidepressant use over the course of 30 days was higher for women than men (17.7 percent versus 8.4 percent), with men ages 18 to 39 having the lowest rate, according to a 2020 report from the Centers for Disease Control (CDC). Antidepressant use shot up for women between 2009 and 2018, but rates among men were flat during the same time period. 

Male aversion to antidepressants isn’t all that surprising. Besides the multitude of reasons why anyone avoids clinical intervention — stigma, fear, shame — many men may avoid antidepressants because of one potential side effect: erectile dysfunction. That’s precisely what makes over-the-counter alternatives attractive, particularly for those who suffer from less severe forms of depression including seasonal affective disorder (SAD) and shorter dips in mood that may not require a clinical-strength boost. They’re also an interesting prospect for people hesitant to add another medication with uncomfortable side effects to their routine.

Over-the-counter (OTC) medications aren’t backed by the same research, but they can help ease mild to moderate symptoms of depression, according to Dr. Mikhail Kogan, medical director of George Washington University’s Center for Integrative Medicine. 

“For a psychiatrist who [didn’t] have training in evidence-based integrative practice, for anxiety or depression they would probably start straight away with medications or therapy and then [up the dosage] at a later time depending on how severe the symptoms are,” said Dr. Noshene Ranjbar, an associate professor of psychiatry at the University of Arizona College of Medicine in Tucson. 

According to the National Institutes of Health (NIH), the most commonly prescribed antidepressants, selective serotonin reuptake inhibitors (SSRIs) like Zoloft and Prozac, prevent neurons from reabsorbing neurotransmitters and can help regulate mood. Anti-anxiety drugs, like Benzodiazepines, throttle the central nervous system, which helps reduce symptoms like extreme worry and panic attacks according to the Center for Addiction and Mental Health (CAMH).

According to a mountain of clinical research, these types of medications work really well for those with severe depression symptoms. The data regarding their long-term effectiveness, though, is more mixed according to a 2022 study published in the journal PLoS. 

Though no one is suggesting you chuck your Zoloft prescription in favor of a bunch of supplements, Kogan said St. John’s wort has been used for mood regulation for centuries, and St. John’s wort and valerian, as well as supplements like omega-3 fatty acids and 5-hydroxytryptophan (or 5-HTP), can treat some mild depression symptoms. That said, more research is necessary to understand the risks of taking them instead of SSRIs or other chemical interventions, and they are not recommended for those with severe depression. They may be a path to avoiding doctors and potential sexual performance issues, but there are obvious downsides, including risk for serotonin syndrome or reduced efficacy of other medications. 

Ranjbar said other go-to supplements that work reasonably well for anxiety and depression include lavender oil capsules like Lavella or Silexan and L-theanine, an amino acid and an active ingredient in green tea. According to a 2021 review and meta-analysis conducted by the Tehran University of Medical Sciences of over 342 screened studies, lavender in capsule formation significantly aided in symptoms of depression compared to a control group.

“[For] L-theanine, the best research for it is for kids with ADHD who also have sleep problems,” said Ranjbar, referring to research done at the University of British Columbia on boys between the ages of eight and twelve. “We use it to help improve sleep because it allows for a little bit of a calming effect without making people sedated. Some people with anxiety can also benefit from L-theanine because it’s mild and not a super-intense sedating drug,” said Ranjbar, referencing mild cases of both anxiety and depression. 

For some folks, symptoms of anxiety and depression may stem from nutritional deficiencies, especially low levels of vitamin B, folate, and omega-3 fatty acids, according to the Indian Journal of Psychiatry. Kogan said broad-spectrum micronutrients are necessary for building neurotransmitters like dopamine and serotonin, so supplements like B complex and folate may help. Researchers out of the California Institute of Behavioral Neurosciences & Psychology have also concluded that vitamin B-12 can help delay the onset of depression symptoms and improve the effects of antidepressants when taken in conjunction. 

The body is also incapable of producing omega-3 fatty acids, a nutrient necessary for abating inflammation and blood clotting, on its own. A 2019 study published in Translational Psychiatry found that omega-3 fatty acids helped mitigate symptoms of depression, especially in the test groups that were given pure EPA (one of several omega-3 fatty acids) as opposed to DHA (another one). Rich in omega-3s, foods like salmon and nuts are some of the best sources of it, but adding a supplement can help fill in where diet falls short. 

Before this all starts to sound too woo-woo, it’s worth noting that effectiveness of OTC remedies varies, and they’re not subject to the same types of regulations as prescription drugs. Ranjbar said websites like ConsumerLabs.com are helpful resources to verify which supplements have undergone third-party testing and follow good manufacturing practices. 

While it sounds appealing to replace prescription meds that come with a whole host of unpleasant side effects with a more natural option, are these OTC mood boosters as effective as your run-of-the-mill Zoloft or Ativan? Not exactly. A 2018 meta-analysis conducted by psychiatrists and researchers in Germany, Switzerland and Texas, and published by the Cambridge University Press, found that 5-HTP, depending on the study, was either not substantially effective or on par with a standard antidepressant. Ultimately, experts say it depends on a number of personal and clinical factors.

Kogan said, for instance, if an individual is trying to wean off antidepressants, including an herbal or dietary supplement may help someone deal with the accompanying withdrawal symptoms. 

But for some individuals, said Ranjbar, OTC herbal and dietary supplements can help punch up traditional prescription antidepressants, especially if their symptoms are not getting better or they need something stronger to be adequately managed. 

Supplements can be used alongside prescription drugs or therapies like cognitive behavioral therapy (or CBT), said Dr. Debra Kissen, a cognitive behavior therapist specializing in anxiety and author of Break Free From Intrusive Thoughts: An Evidence-Based Guide for Managing Fear and Finding Peace.

Avoiding the annoying aspects of the health care system could be a perk of OTC options, but every expert interviewed for this story emphasized that people shouldn’t explore alternative therapies totally unsupervised. 

“Sometimes when people are looking for over-the-counter solutions, it’s because they’re trying to figure it out on their own versus actually getting help,” said Kissen. “As long as you’re doing it in this way [of recognizing] you deserve help and support, and [supplements] are just a tool, that’s fine… But it shouldn’t be a lonely process.”
